What is Azithromycin?

Azithromycin is a macrolide antibiotic — it kills the bacteria causing your infection. Think of it as specialized backup for your immune system. Your body is already fighting; azithromycin makes sure it wins, especially against bacteria that other common antibiotics might miss.

It's one of the most prescribed antibiotics worldwide because it works against a wide range of infections and is well-tolerated by most people. Unlike penicillin-based antibiotics like amoxicillin, azithromycin is effective even if you're allergic to penicillin. Plus, it has a long half-life — meaning it stays in your system for days after you stop taking it. That's why the famous Z-Pack (five-day course) works so well.

How to Take Azithromycin

Azithromycin comes in different dosing schedules depending on your infection. Here's what you need to know:

Common Use Dosing Schedule
Z-Pack (most common) 500mg day 1, then 250mg daily for 4 more days (5 days total)
Chlamydia (STI) 1000mg (1g) as a single dose

Important tips for taking azithromycin:

  • Take it at the same time each day
  • You can take it with or without food (food helps if stomach upset)
  • Finish the entire course, even if you feel better
  • Don't skip doses or take extra doses if you forget one

Missed Dose?

Take it as soon as you remember. But if it's close to the time of your next dose (within a few hours), skip the missed one and continue with your normal schedule. Never double up on doses. Can I take azithromycin with food?
Yes, you can take azithromycin with or without food. Taking it with food can actually help prevent nausea and stomach upset. Just make sure to take it at the same time each day so you stay on schedule and don't miss doses.
Why is azithromycin prescribed instead of amoxicillin?
Azithromycin is often chosen when someone is allergic to penicillin (which amoxicillin is based on). It also covers some bacteria that amoxicillin doesn't, works exceptionally well against certain STIs like chlamydia, and has the advantage of staying in your system longer — letting you take fewer doses over a shorter time. How long does azithromycin stay in your system?

This is also why you shouldn't take extra doses if you miss one — your body already has enough medicine in it, and it's still active and working.
Can I drink alcohol while taking azithromycin?
There's no direct chemical interaction, but alcohol can worsen nausea and stomach upset — and azithromycin can already cause these side effects. It's best to avoid alcohol during your treatment. If you do drink, keep it minimal and watch how you feel. If you experience increased nausea or stomach problems, stop drinking and call us.
