I'm a Nurse Practitioner, not a salesman. Strep is usually straightforward, but a few patterns mean you need to be seen in person right away, not texting me.

Head to the ER if any of these apply:

  • Trouble breathing, drooling because you can't swallow, or a muffled "hot potato" voice, can be a sign of an abscess or airway swelling
  • Severe one-sided throat swelling, or you can't open your mouth fully (trismus)
  • Stiff neck along with a high fever, rule out meningitis
  • A sandpaper-feeling rash with high fever, could be scarlet fever needing prompt evaluation
  • Severe dehydration because you can't keep fluids down
  • A child under 3 with high fever and significant throat pain

Common Questions

Can strep throat be treated without a swab?

Yes, in many cases. The modified Centor criteria let a Nurse Practitioner gauge the likelihood of strep based on symptoms: sudden severe sore throat, fever, swollen tender lymph nodes, white patches on the tonsils, and the absence of cough. When the picture is clear, treatment can be started by text. If the picture is murky, Chris will ask you to grab a $10 rapid strep test at a nearby pharmacy or urgent care first, no charge for the honesty.

What if it's not strep, just a viral sore throat?

Most sore throats are viral. If your symptoms point that way (cough, runny nose, hoarseness, mild fever), Chris will say so and walk you through symptom relief, fluids, throat lozenges, ibuprofen, salt-water gargles. He won't hand you antibiotics you don't need. If something else is brewing like mono, an abscess, or post-nasal drip, he'll tell you what to do next.

What antibiotic will I get?

First-line therapy for strep is penicillin or amoxicillin. They've been the standard for decades, the bacteria still respond well, and they're cheap. If you're allergic to penicillin, Chris uses an alternative like azithromycin, cephalexin, or clindamycin depending on your history and how severe the allergy was.

How much does the visit and the antibiotic cost?
