Medication Guide — Patient Education

What is Hydroxyzine?
Hydroxyzine is a prescription-strength antihistamine — think of it as a much stronger version of Benadryl. It blocks the histamine response that's causing your hives, itching, and swelling. Your body releases histamine when it overreacts to something (an allergen), and hydroxyzine stops that reaction in its tracks.
The medication comes in two forms: Atarax (hydroxyzine hydrochloride) and Vistaril (hydroxyzine pamoate). They work the same way but are absorbed slightly differently. Your provider chose the specific form and dose that's right for your situation. Hydroxyzine has been used safely for over 60 years, and doctors trust it because it works fast and reliably.
One important thing to know upfront: hydroxyzine makes you drowsy. That's not a side effect — it's part of how it works. This drowsiness actually helps calm anxiety and itching. But it means you need to plan your life around taking it, especially when you first start.
Conditions This Treats
Hydroxyzine is prescribed for allergic reactions and related conditions. Here are the main uses:
Sometimes used for acute anxiety before procedures or during panic. Not for long-term anxiety management.
Itching (severe)
Reduces severe itching from various causes, including eczema or allergic dermatitis.
Allergic Reactions (itching/swelling)
When standard antihistamines aren't strong enough, hydroxyzine works fast and effectively.
How to Take It
Taking hydroxyzine correctly is important for effectiveness and safety. Here's what you need to know:
Detail
Instructions
Typical dose
25-50mg per dose
How often
Every 6-8 hours as needed
With food?
Can take with or without food
Best time to take it
When you don't need to be alert — sitting at home, before bed, or when you can rest
How quickly it works
Starts working in 30-60 minutes. Peak effect at about 2 hours.
Important
DO NOT DRIVE or operate machinery until you know how it affects you. Drowsiness is significant.
Drowsiness is a major effect — plan accordingly. Most people feel noticeably drowsy within 30-60 minutes. This is significant enough that you shouldn't drive, operate machinery, or do anything requiring alertness until you know how your body responds. Some people adapt after a few days; others remain quite drowsy. Plan your dose for when you can rest.
Side Effects
Most people tolerate hydroxyzine well, but there are side effects to watch for:
Common (expected)
Drowsiness (very common — this is the main effect)
Dry mouth
Dizziness
Headache
Blurred vision
Fatigue
These effects usually appear within 30 minutes and can improve as your body adjusts. The drowsiness often persists, which is why timing your dose is important.
Serious (seek help)
Severe drowsiness or unresponsiveness — can't be awakened
QT prolongation (rare) — chest pain or irregular heartbeat
Difficulty urinating — inability to pass urine
Confusion or extreme disorientation
Severe dizziness or loss of balance
Signs of allergic reaction (despite taking the medication)
Stop taking hydroxyzine and contact us immediately if you experience any of these. These are uncommon but need attention.
Stop Hydroxyzine & Seek Emergency Care If:
Difficulty breathing — worsening allergic reaction despite medication — this suggests the medication isn't working and you need different treatment
Severe drowsiness or can't be awakened — overdose risk
Chest pain or irregular heartbeat — possible QT prolongation
Inability to urinate despite full bladder — urinary retention
Extreme confusion or severe disorientation
If in doubt, text us or call 911. Don't wait it out. We're available for questions anytime.
Important Warnings & Interactions
Let your provider know about these situations before starting hydroxyzine:
Alcohol — CRITICAL
Avoid alcohol completely while taking hydroxyzine. Alcohol amplifies the drowsiness effect significantly and can make you dangerously sedated. The combination impairs judgment and coordination. This is a serious interaction, not a minor concern. Skip alcohol entirely while on this medication.
CNS Depressants
Don't combine hydroxyzine with other medications that cause drowsiness — opioids, benzodiazepines, sleeping pills, or muscle relaxers. The combined effect can be dangerous. Talk to us before taking any new medication.
Other Antihistamines
Don't combine hydroxyzine with Benadryl or other antihistamines. They both block histamine and combining them increases side effects without added benefit. If you're still itching, call us to adjust your dose rather than adding medications.
Anticholinergic Medications
Mention if you take medications for overactive bladder, IBS, or asthma. Combining these with hydroxyzine increases the risk of urinary retention and other side effects.
Pregnancy & Breastfeeding
Tell us if you're pregnant or breastfeeding. Hydroxyzine is generally considered safe during pregnancy for serious allergic reactions, but it's not a first choice. The decision depends on your specific situation. It passes into breast milk in small amounts.
Elderly Patients
If you're over 65, hydroxyzine can cause confusion and falls due to drowsiness and dizziness. Your provider may use a lower dose. Be especially careful with balance and avoid driving until you know how it affects you.
Storage & What to Do If You Miss a Dose
Storage
Store at room temperature, away from moisture (not the bathroom)
Keep in original container
Keep away from children and pets — hydroxyzine can be dangerous in overdose
Don't share with others, even family members
Missed Dose
For allergy symptoms — take it immediately when itching/hives start
Never double-dose to make up for a missed one
Space doses out — aim for doses about 6-8 hours apart
Set phone reminders or use a pill organizer
Text us if you're struggling with the routine
Frequently Asked Questions
Here are answers to the questions we hear most from patients taking hydroxyzine:
Drowsiness is one of the main effects of hydroxyzine — it's actually why it works so well for anxiety and allergies. Most people feel noticeably drowsy within 30-60 minutes. It's significant enough that you shouldn't drive or operate machinery until you know how it affects you personally. Some people adapt after a few days; others remain quite drowsy. Take it when you don't need to be alert.
No — avoid combining hydroxyzine with other antihistamines like Benadryl. Both are antihistamines and combining them increases drowsiness and other side effects without better results. If you're still having allergic symptoms despite hydroxyzine, call us before adding anything else. We can adjust your dose or switch medications rather than combining them.
Hydroxyzine usually starts working within 30-60 minutes. You'll likely feel drowsy first, and then the itching and hives should begin improving. Peak effect is usually around 2 hours. Take it when you have time to relax or before bed if drowsiness is a concern.
No — absolutely avoid alcohol while taking hydroxyzine. Alcohol amplifies the drowsiness effect significantly. The combination can make you dangerously sedated and impair your judgment and coordination. Skip alcohol completely while you're on this medication. This is a serious interaction, not a minor concern.
Hydroxyzine is not addictive in the traditional sense. Unlike benzodiazepines (like Xanax), it doesn't cause dependence or withdrawal when you stop taking it. You won't develop tolerance or crave it. That said, if you've been taking it regularly for anxiety, you might notice anxiety symptoms return once you stop. That's the underlying condition returning, not addiction. It's safe to use as prescribed without addiction concerns.
