Red flags: when this is an emergency

Most abscesses are miserable and still local. A few become a race against the airway. I want this list in your pocket. If you meet it, you do not text me first. You go.

I can swallow, but my cheek is puffy. Do I need the ER?
Chris
Puffy over one tooth, swallowing fine, breathing fine: get a dentist today if you can. If the puff climbs, the tongue rises, or spit is hard to swallow, that is the ER.
Red warning card titled Go to the ER or call 911, do not text first. Six red flags with icons: trouble breathing, cannot swallow spit, floor of the mouth rising, neck hard or swelling, eye swelling shut, and high fever with confusion.
If you see any of these, skip the text. Go to the ER or call 911.

Why the floor of the mouth matters

Under your tongue is a soft space. If infection fills it, the tongue rides up and back. That can close the airway. This pattern is the one emergency teams watch for with dental infections. I will not manage it from a phone.

A hard, rapidly spreading neck is the same family. Do not ice it and hope. Do not wait for morning clinic.

Same-day dentist, not later this month

  • One tooth is tap-tender and the gum has a boil.
  • Swelling stays over that tooth and you can still swallow.
  • You feel a bad taste and the pain is getting louder each night.
  • You have a low-grade fever and you still look like yourself in the mirror.

What a text visit is not for

I will not talk you out of the ER. If your story is messy and I cannot see your airway, I send you in. You do not pay for a $59 visit when I tell you text care is not safe.

Next stop is calmer on purpose: what care looks like when you are stable. The dentist plan. The bridge plan. The honest limits of medicine.

Questions I get

I have a fever of 101 and a puffy gum. ER or dentist?

If you can swallow and breathe and the swelling is local, a same-day dentist or an urgent dental clinic is the usual door. If the fever is high, you feel confused, or swelling is spreading, use the ER.

Can I wait overnight if the pain is only a throb?

A throb without spreading or swallow trouble can wait for morning dental care. Recheck the red flags if you wake worse.

Should I text you from the ER parking lot?

If you already know you need the ER, go in. You can text later for follow-up once you are stable and back in a state I can treat.

Soft ask: text me the tooth, the swelling, and whether you can swallow. I will tell you if this is me, a dentist, or the ER. A consult does not guarantee a prescription.
